Breakthrough Treatments

Although this blog began as a focus on breast cancer, it has led me down many research paths around conventional and non-conventional treatments. The sad reality of this disease is that so many patients struggle to access the right treatments. Insurance coverage is focused on the conventional therapies and those cutting edge or more natural therapies are typically expensive, making them unavailable to the majority of patients. The fact that people have to choose between providing for their family or getting the best care shows how incredibly broken our medical industry is. One advantage of this digital era is that social media is bringing more and more awareness to these breakthrough treatments. The amount of advocacy is so promising in changing how we approach this disease. One thing I promised myself in this journey is that I would help become a voice for others through education and advocacy.

This week, insurance coverage for histotripsy treatments was approved in the state of Michigan. What is that? Histotripsy is a non-invasive, non-thermal ablation technique that uses focused ultrasound pulses to mechanically destroy tissue at a cellular level. Unlike thermal ablation methods (e.g., radiofrequency ablation, microwave ablation), histotripsy causes tissue destruction through acoustic cavitation—generating microscopic bubbles that collapse and liquefy targeted tissue.

Histotripsy is a treatment for primary liver cancer (hepatocellular carcinoma) and secondary (metastatic) liver tumors. It is of particular interest because it offers several potential advantages. The treatment avoids heat-based damage to nearby structures such as bile ducts and vessels. There are no incisions or probes which accelerate recovery and reduce infection risk. Precise ablation allows for millimeter precision destruction that leaves surrounding healthy tissue unharmed. Evidence also shows that treatment may release tumor antigens due to stimulation of an immune response.

Treatments like this represent a shift to less invasive, precision oncology that improves quality of life for patients. They provide options for those who may not be candidates for surgery due to comorbidities, giving them curative treatments versus palliative care. As treatments such as this become portable and user-friendly, it can be offered outside of tertiary care centers, increasing equity to access in all communities. They provide a transformation in cancer care philosophy, emphasizing minimally invasive, immune-conscious, and patient-centered approaches. Their broader impact lies in extending life, enhancing quality of life, and redefining what is possible in oncology. We need to keep driving visibility on these treatments as they reach clinical trials and advocate for insurance coverage.
